SERVICE DESCRIPTION

 

Mid Hampshire Healthcare (a GP Federation of 19 practices)  piloted a Tier 2 Community Cardiology Service for West Hampshire CCG in October 2016. This was initially a 15 month programme. Due to the success of this pilot the service has continued and is now entering into an 8th year.

Mid Hampshire Healthcare (MHH) is contracted by the Hampshire, Southampton and Isle of Wight Clinical Commissioning Group (CCG) to provide a community based service for patients requiring cardiology investigation and treatment following referral of patients from general practice.

 

The aim is to provide a more convenient local service, reduce waiting times and improve service quality whilst reducing NHS service spend.

 

The Community Cardiology Service (CCS) is delivered by Watercress Medical a GP partnership in Medstead which is a member practice of the Mid Hampshire GP Federation.

The Community Cardiology Service works closely with hospital providers such as Hampshire Hospitals NHS Foundation Trust and University Hospital Southampton NHS Foundation Trust. Our hospital colleagues provide valued support and advice for the community service.

The Community Cardiology Service is led by Dr Peter Savill and all service administration is based at Watercress Medical.​
